The Reason You Wake Up Stiff Is Not Under Your Back. It Is Under Your Head.

What "neutral" means, and why your spine cares

Stand up straight and your spine holds a gentle natural curve. Sleep researchers call that position the neutral spine, where your head, neck and back sit in the same relaxed line you have while standing.

A pillow that has gone flat lets your head sag below that line. A tall, fluffy one props it above the line. And if you sleep on your side, there is a gap between your ear and the mattress that a flat pillow cannot fill, so your head tips down toward your shoulder all night.

Your spine is one connected chain. Bend the top of it for eight hours and the tension does not politely stay in your neck. It travels down. That is why the ache so often shows up in the lower back, and why it is worst in the first minutes after you get up.

What researchers found after a full year

In a 12-month study published in the Journal of Surgery in 2025, researchers followed 70 people with chronic neck pain who switched to an ergonomic cervical pillow, the kind shaped to hold that neutral line instead of leaving it to chance. They measured pain, neck disability and sleep quality, then split the results by age.

Here is the finding almost nobody talks about. The group over 50 improved the most.

The researchers recorded those results as statistically superior to the ones seen in the younger groups.

Read that again, because it says the opposite of what you have probably been telling yourself. The people who had been carrying this the longest, the ones most likely to write it off as age, were the ones whose bodies responded the most once the neck was finally supported through the night.

  1. You wake stiff in the neck or upper shoulders, not only in the lower back.
  2. The stiffness eases within an hour of getting up and moving around.
